The short answer
Without insurance, manufacturer self-pay offers for brand-name GLP-1 medicine start at $149 a month as of October 2026. Prices depend on the drug, dose, eligibility and refill timing; higher-dose Zepbound can reach $699 if the on-time refill terms are missed. If an online clinic prescribes it, add any membership or visit charges.
| Medicine | Form | Starting dose | Maintenance doses | Where |
|---|---|---|---|---|
| Wegovy pill | Daily tablet | $149 | $299 | NovoCare Pharmacy |
| Foundayo | Daily tablet | $149 | $299 | LillyDirect |

| Zepbound | Weekly injection | $299 | $449** | LillyDirect |

Cheapest GLP-1 without insurance: compare the full cost
The lowest advertised starting price here is $149 for the 1.5 mg Wegovy pill or the 0.8 mg Foundayo pill. Starting doses are not the final monthly cost for many patients: later pill doses can cost $199–$299, and injections can cost more. Compare the dose your clinician expects you to use, not just the first fill.
If you already have a prescriber, manufacturer self-pay programs can avoid a clinic membership. If you need an online clinician, compare the medicine price plus visit fees or membership fees. Our no-membership guide covers pay-per-visit options; the online provider comparison covers ongoing clinic plans.
Zepbound self-pay prices
LillyDirect sells Zepbound to self-pay patients as single-dose vials or a multi-dose KwikPen at the same price. A "month" is 28 days: four vials or one KwikPen. Needles or pen needles are bought separately.
| Dose | On-time refill | Regular price |
|---|---|---|
| 2.5 mg (starting dose) | $299 | $299 |
| 5 mg | $399 | $399 |
| 7.5 mg | $449 | $499 |
| 10 mg | $449 | $699 |
| 12.5 mg | $449 | $699 |
| 15 mg | $449 | $699 |
The $449 price for 7.5 mg and up depends on Lilly's Zepbound Journey Program, which requires a refill within 45 days of the last one. Missing that window can add $250 a month at 10 mg and above.

Pills are cheaper while you're on the lower doses, but at maintenance doses they cost about the same as the cheapest injections. Read Wegovy pill vs Foundayo.
What a first year costs
Doses go up gradually, usually every four weeks, so the first few months are cheaper. Here's a rough first-year total if you step up one dose a month to a common maintenance dose and pay manufacturer self-pay prices (12 fills, no clinic membership):
| Medicine and target dose | First 12 months | With a $149/mo clinic |
|---|---|---|
| Wegovy pill to 25 mg | $3,338 | $5,126 |
| Foundayo to 17.2 mg | $3,188 | $4,976 |
| Wegovy injection to 2.4 mg | $3,888 | $5,676 |
| Zepbound to 10 mg | $5,188 | $6,976 |
Zepbound is billed in 28-day fills, so a real year has 13 fills, not 12. Try your own numbers in the cost finder.
If you have insurance
Coverage for weight loss varies widely. Many employer plans exclude "anti-obesity medications" or require prior authorization, documentation of your BMI and sometimes proof you tried diet and exercise first. Coverage is more likely when the medicine is prescribed for an approved medical use:
- Wegovy: also approved to lower the risk of heart attack and stroke in adults with heart disease and overweight or obesity.
- Zepbound: also approved for moderate to severe obstructive sleep apnea in adults with obesity.
Manufacturer savings cards can bring copays down to as little as $25 a month for commercially insured patients whose plan covers the drug. Self-pay programs generally can't be combined with insurance or used with government coverage such as Medicare or Medicaid; check each program's terms.
Ways to pay less
- Refill on time. Lilly's best Zepbound and Foundayo prices need a refill within 45 days.
- Start new. Wegovy's $199 introductory fills are for new patients only.
- Prepay the clinic, not the drug. Annual clinic memberships run about half the monthly rate. Only do this if you're confident you'll continue.
- Use HSA or FSA funds. Prescription medication is often eligible; check your plan.
- Ask about the lowest effective dose. Some people maintain well below the top dose, which can cost less, especially with Wegovy pills and Foundayo. That's a decision for you and your clinician.
